Use our convenient, online tool to pre-register for childbirth or any inpatient or outpatient procedure or service.
To begin, select the the hospital where you will have your procedure or service from the options below.
Use this option to pre-register for childbirth (labor and delivery). All required forms should be submitted by the seventh month of pregnancy.

Inpatients are pre-admitted to the hospital through their physician or admitted through the hospital's Emergency Room. If you have questions about our admitting process, please contact the Admitting Office at (713) 704-0446.
Please note that outpatients are encouraged to pre-register at least three days prior to an outpatient procedure.
Use this option to pre-register for a surgery, procedure or service, such as Inpatient Hospital Stay, Outpatient Surgery, Day Surgery, Endoscopy, Diagnostic Test, Ultrasound, Mammogram.
