The new Medication Therapy and Wellness Clinic focuses specifically on anticoagulation management, immunizations, pharmacotherapy review, and tobacco cessation. Clinic pharmacists facilitate care through the evaluation of patients and modification of therapy using medical staff-approved protocols.
Upon physician referral, clinical pharmacist services are available for outpatients, Monday and Thursday, 7:30 a.m. to 4 p.m. at the Medication Therapy and Wellness Clinic located in Suite 100 of Memorial Hermann Southeast’s Medical Plaza II on 11920 Astoria Blvd. For more information, call 281-929-4227.
